Fall-Risk and In-Home Safety Assessment and Accessibility Assessment 

Age-Friendly Innovators Inc. (AFI) is committed to helping older adults stay in their own homes and communities safely, for as long as possible.  Each year in America millions of older adults fall. One in three older adults falls each year but less than half tell their doctor–even though a single fall doubles your chance of falling again. (Source: Centers for Disease Control and Prevention)

Falls are Serious and Costly

  • One third of Americans age 65+ falls each yearwitch falling
  • Every 13 seconds an older adult is treated in an emergency room  for a fall
  • Every 20 minutes an older adult dies from a fall
  • Falls are the leading cause of fatal injury and the most common cause of nonfatal trauma-related hospital admissions among older adults

With a grant from the southern Oregon-based Chaney Family Foundation, Age-Friendly Innovators developed a Fall-Risk In-Home Safety and Accessibility tool  for evaluating homes and their older adult occupants and providing recommendations for preventing falls. The assessment is based on the individual’s fall risk profile using Centers for Disease Control and Prevention (CDC) criteria combined with an analysis of those elements in the home environment that increase/decrease the risk of falling. The assessment process takes 60-90 minutes and is currently done at no cost to the resident by AFI staff certified as Aging-in-Place Specialists (CAPS) by the National Association of Home Builders.

With the Chaney Family Foundation grant and a sub-grant through the Oregon Department of Human Services, AFI  conducted over 60 assessments in 2015 and is actively engaged in  on  turning the assessment tool into an iPad /iPhone app for use by families and caregivers.
